I knowingly, and without duress, do voluntarily register for and participate in Qigong, meditation, and integrative wellness classes, programs, and/or workshops, being offered by Ann Duvall. I understand that during the class, program, workshop, I will receive information and instruction about Qigong, meditation and integrative wellness. I recognize that participation may require physical exertion, which may be strenuous and may result in accidental injury, and I am fully aware of the risks and hazards involved. I also understand that if at any time during any class or when receiving instruction that I feel discomfort or strain, it is my responsibility to cease the activity and notify the instructor. If I do not inform the teacher/instructor of any physical discomfort or strain, I have consciously chosen not to and I take full responsibility for my choice. I understand I may rest at any time during the class. By attending this class, I agree that I am solely responsible for my health and well-being, as well as the decision to practice in qigong and meditation, and integrative wellness.

 

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in Qigong, meditation and integrative wellness classes, programs, and workshops. I represent and warrant that I am physically and mentally fit and I have no medical condition that would prevent my full participation in these classes, programs, and workshops. I hereby assume all risk of physical and mental injuries, disabilities, and losses which may result from or in connection with my participation in these classes, programs and workshops. Acting for myself, heirs, personal representatives, and assignees, I do hereby release Ann Duvall from any and all claims, actions, suits and controversies at law or in equity by reason of any matter, cause or thing whatsoever that I may sustain as a result of or in connection with my participation in the Qigong, meditation and integrative wellness classes, programs, or workshops that I have chosen to attend. I have read and fully understand the waiver listed above. I voluntarily agree to the terms and conditions stated above.
